If you are requesting a withdrawal for medical purposes, you must also complete a Medical Leave Form form prior to your meeting.

To determine eligibility for housing accommodation, students must provide documentation of disorders from the diagnosing physician or other professional.

To determine eligibility for services, students must provide current documentation of diagnosed psychiatric and psychological disabilities.

The Academic Support & Enrichment Center provides services to students with diagnosed psychiatric and psychological disabilities.
