Title IX Reporting Form

This form is used to report information regarding possible violations of Denison’s Sex Discrimination and Sexual Misconduct Policy to the Title IX Coordinator. You may use this form to report an incident that happened to you or an incident that involved other members of our community.

Please fill out as much information as you can. Once the Title IX Coordinator receives this report, they will reach out to the person identified as the complainant to offer support and to discuss resolution options. You can find a list of some possible supportive measures and a description of the formal and alternate resolution processes in the policy linked above.
