The online service request form should be used to report issues to Facilities Services for individuals who do not have a Denison account. Individuals with a Denison account should log their issue via TOPdesk in MyDenison. If the issue is urgent, please contact the Service Desk by phone at 740-587-6264 between 7:30am - 4:00pm.
