Transferring Your Health Care
While at Denison, if you would like to receive care for a chronic condition or get medication refills, simply sign a Release of Information form to give your home provider permission to share protected health information with our team.
Information commonly shared includes:
- Most recent visit note, related to the diagnosis
- ADHD test results
- Medication name, dose, and frequency
- Current & past therapies
- Recommendations for Plan of Care while at school
