All medical and behavioral health information provided to the Hoaglin Wellness Center is confidential and will not be released without student permissions.

All medical and behavioral health information provided to The Wellness Center is confidential and will not be released to anyone without a signed release of information from the student.

Professional codes of ethics, state and federal laws and professional standards regulate the rules of confidentiality. As such, Denison’s Health and Counseling Services follow these standard codes of conduct and safeguard the “protected medical and behavioral health records” of our students.

Without a written release of information, professional staff cannot speak with parents, administration, faculty, or friends about a student’s medical and behavioral health history or visits to The Wellness Center. The only exceptions to the rules of confidentiality are if an individual experiences imminent danger to themselves or someone else, or circumstances involving the neglect and/or abuse of a child (under 18) or an elderly person.

Records of students’ use of The Wellness Center will not be placed in their college records, placement files, or official transcript.
